Office Policies

Please notify us immediately of changes to your insurance, name, address, pharmacy, phone and email.

If you need to cancel an appointment, please contact us at least 24 hours in advance. Patients who do not cancel and do not not show up for their appointments will be asked to find a new doctor. ​

All patients must request refills on the patient portal or by calling their pharmacy. Please allow us 24-48 hours to refill your prescriptions.

An office visit is required for all antibiotics, pain medications and other controlled substances. ​

We do not treat chronic pain and ADD/ADHD, and we do not prescribe benzodiazepines such as Ativan and Xanax. Patients will need to see a specialist for these conditions.

Rude and disrespectful patients will be immediately dismissed from the practice. We have zero tolerance for this type of behavior.
